MASTER SERVICES AGREEMENT - SUBCONTRACTOR SERVICES Business Name: Dope Restoration Operating Entity / DBA: To be confirmed before production use. 1. Independent Contractor. Vendor is an independent contractor and is responsible for its own labor, tools, insurance, taxes, safety, means, methods, and code compliance. 2. Scope Control. Vendor may perform only the work authorized in a signed work order or signed change order. Verbal instructions, text messages, or field discussions do not authorize additional payment unless converted to a written change order. 3. Insurance and Licensing. Vendor must provide a current certificate of insurance before scheduling work and must maintain any license, registration, or permit required for its trade or scope. 4. Permits and Inspections. Vendor must identify permit or inspection requirements before work begins. Regulated work may not proceed without the required permit path. 5. Hidden Conditions. If hidden damage, omitted damage, code requirements, or expanded scope is discovered, Vendor must stop the affected work area, photograph the condition, describe the issue, and provide pricing for review before proceeding, except for emergency stabilization. 6. Payment. Payment follows the work order milestones. Final payment requires final invoice, completion photos, inspection proof if required, and lien waiver documentation. 7. Lien Waivers and Supplier Payment. Vendor must provide partial lien waivers with progress payments if requested and a final lien waiver before final payment. Vendor must disclose subcontractors and material suppliers when requested. 8. Workmanship. Vendor warrants its workmanship for the warranty period listed in the work order and will correct deficient work caused by Vendor at Vendor's expense. 9. Property Protection. Vendor must protect the property, clean the work area, and repair damage caused by Vendor, Vendor's employees, subcontractors, or suppliers. 10. Project Documentation. Vendor understands Dope Restoration may retain and use work orders, invoices, photos, inspection records, lien waivers, and change-order documentation as part of the project file.
